Platform Operation Required

Will be requested from the parent. The authorization covers data collection (GPS load, wellness check-ins, performance metrics), SMS notifications to athlete and parent for performance and care purposes, and access to data by Athletic Trainer and authorized coaching staff for athletic care and education. Research Inclusion Optional · Opt-In

Will be offered to the parent as an opt-in. Ticking this only marks the athlete to be asked. The authorization covers whether this athlete's de-identified performance data may be used in research studies on adolescent athlete health. All identifying information (name, date of birth, contact info) is removed from research exports. Consent can be withdrawn anytime; future data will not be included. See the Privacy Notice for the research data handling policy.

Marketplace provider sharing (RD, sports psychology, ortho referral) is consented separately at the time of each request — not at enrollment.
